3 recalls across 2 model years
Recalls 1 and 3 describe the same underlying defect. Treating as a single issue.
The Lexus RX 400h has 3 recalls covering the 2004–2006 model years, with the most serious being a hybrid inverter control board failure on 2006 models that can trigger multiple warning lights, including the brake system warning, while the vehicle is moving. On 2004 models, improperly installed retaining clips on the floor carpet cover in front of the center console can work loose, allowing the cover to shift toward the accelerator pedal and interfere with its movement.
