The Ford E-450 has 32 recalls spanning 2005 through 2027, with the most serious recent issue affecting 2023 models where a rear axle rotor bolt may be insufficiently tightened, allowing a rear wheel to separate from the vehicle while driving.
The newest production years bring several concerns. On 2026-2027 models, the backup alarm connector can work loose and detach, silencing the alarm when reversing and removing the pedestrian warning. On 2022-2023 models, paint along the fuel tank weld seam can peel and expose bare metal to corrosion, eventually producing a fuel leak that can ignite near a heat source.
The 2021-2022 model years carry a combined brake and steering concern: on dual-rear-wheel configurations, a poorly seated connection between the power steering pressure line and the brake booster unit can suddenly dump all the power steering fluid, leaving the driver with heavy steering and reduced brake assist simultaneously. On 2021 models, a rear wire harness can chafe against the frame and damage circuits serving both the fuel pump and the anti-lock brake system, potentially stalling the engine without warning. Also on 2021 models, the rear axle differential may have been underfilled from the factory, risking axle failure and driveshaft separation. A 2021 engine cover issue, where mispositioned insulation allows the interior-facing cover surface to get hot enough to burn skin, required two separate recalls after the first repair attempt did not fully resolve the problem.
On 2018 transit bus conversions, rear axle leaf spring U-bolts may have been undertightened, allowing the axle to shift or separate. On 2017-2018 ambulance and shuttle bus conversions with specific backup camera monitors, the monitor can reset to a mirrored image, causing a driver to steer the wrong direction when reversing. Brake lights on some 2017-2018 ambulance builds can illuminate without the pedal being pressed, confusing following drivers. The 2016 model year has a trailer brake controller on 6.8L-equipped vehicles that may not activate trailer brakes when slowing, extending stopping distances. A 2016 propane-converted variant also has a fuel control valve solenoid that can short and stall the engine while driving.
Propane conversion recalls extend to 2011-2012 model years, where fuel line fittings or valves on converted vehicles can leak propane near ignition sources and create a fire risk.
Wheelchair lift issues account for a large cluster across 2005-2014 model years on vehicles fitted with various Ricon lift systems. Platform side plates can crack and drop the lift platform against the door, lift control software can cause the platform to overshoot floor level and tip a wheelchair occupant, anti-stow interlocks can fail to detect a user and allow the lift to stow with someone on it, and restraint belt systems can allow lift movement before the belt is properly latched. The 2005 model year also has an incorrect tire pressure label on the rear axle placard, where the stated pressure is too low to support the rated axle load.
