On certain vehicles, the front passenger seat occupant detection mat can fatigue during field usage depending on the seat configuration, vehicle geometry, manner and frequency of front passenger entry/exit. Over time micro cracks could develop in the side flanks of the mat, which could lead to a break of the conductive path and the system will recognize a failure and with exception of head protection system the front passenger air bag will be deactivated. The airbag On/Off lamp will be illuminated to make the occupants aware of the deactivation.

Certain full-size passenger, cargo and cutaway vans may have front and rear seat belt buckles that will not latch or will not unlatch. There are two conditions that render the buckle inoperative after customer usage. Specifically, (1) with a damaged ejector hood, the latch tongue cannot fully enter the buckle and will not latch, and (2) if the release bar moves permanently out of position the buckle will either be unable to engage the latch or the pushbutton will be unable to unlatch the buckle.

Certain vehicles equipped with remote start capability fail to comply with the requirements of CMVSS 208 - Occupant Restraint Systems. The safety belt reminder light for the driver's seating position does not illuminate after the ignition key is inserted and turned to the 'run' position following a remote start. As well, the audible chime for the driver's seating position will not sound if the driver and a front seat passenger enter the vehicle following a remote start, the passenger buckles his/her seatbelt within 25 to 33 seconds after the remote start, and the ignition key is then turned to the 'run' position. Note: This non-compliance condition has no affect on vehicle safety.

Dealers will recalibrate the Sensing Diagnostic Module (SDM) to activate an enhanced seatbelt reminder feature, which repeats the chime and telltale cycles up to two times if the seatbelt is not buckled and if the vehicles speed is over 8 km/h. This enhanced feature is already activated on 2007 vehicles.
On certain Crew Cab vehicles, there may not be sufficient webbing on the retractor spool for the rear center seatbelt to release from the automatic locking mode (ALR). To release the ALR function and return to emergency locking retractor mode (ELR), the seatbelt has to be spooled all the way in. Should the seatbelt not switch back to ELR, it would be rendered unusable. An unbelted seat occupant would receive increased injury during a vehicle crash.

Certain King Cab vehicles. Wires in the harness routed through the rear doors and into the body may break due to over bending of the harness as the door is opened and closed. On the left side, the harness contains wires for the driver seatbelt pretensioner, and on the right side the harness contains wires for the front passenger seatbelt pretensioner and for the seatbelt tension sensor, which is part of the occupant classification system. Wires for the rear audio speakers are also contained in the harness on each side. If the wires for the seatbelt pretensioner and speaker break and make contact, there is a possibility that the pretensioner may deploy. If this happens when the seatbelt is retracted, it will not be possible to use the seatbelt. If one of the wires in the right side harness for the seatbelt tension sensor breaks, the passenger side front airbag will not deploy as designed in a frontal crash.

If this happens, the seat back may recline until it makes contact with an object behind the seat, such as the 2nd row seat cushion. The occupant could potentially lose contact with the steering wheel or foot pedals during such an event.
On certain GS 300, GS 300 AWD, GS 430, IS 250, IS 250 AWD and IS 350 vehicles, the front seat belt retractors may contain an inappropriately sized part for the emergency locking system. This can prevent the locking system from releasing when it is activated. If the seat belt is locked in this condition, the occupant may either not fasten the seat belt, or be inadequately restrained due to improper belt-fit thereby increasing the risk of injury.
