On certain vehicles, the Wireless Ignition Node (a.k.a ignition switch) may allow the ignition key to inadvertently move from the ON position to the accessory (ACC) position while driving, causing the engine to shut off. Loss of propulsion, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash causing property damage and/or personal injury.
On certain vehicles, if the driver's all-weather floor mat is not secured by the retaining clips, or is placed on top of an existing carpeted floor mat, the mat could move forward and interfere with the accelerator pedal. If this occurs, the accelerator pedal could become stuck in the wide open position. A fully depressed accelerator pedal may result in very high vehicle speeds and make it difficult to slow or stop the vehicle. This could result in a crash causing property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will modify or replace the accelerator pedal, as well as the genuine Lexus all-weather floor mat.
On certain vehicles, if the forward retention clip used to secure the floor carpet cover, which is located in front of the center console, is not installed properly, the cover may lean toward the accelerator pedal and interfere with the operation of the accelerator pedal arm. If this occurs, the accelerator pedal could temporarily become stuck in a partially depressed position, rather than return to the idle position. This could result in a loss of throttle control and a crash causing property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will replace the floor carpet cover with a newly designed version.
On certain AWD vehicles, if the floor carpet near the accelerator pedal is not reinstalled correctly following a service repair, the plastic pad embedded into the floor carpet could interfere with the operation of the accelerator pedal. If this occurs, the accelerator pedal could temporarily become stuck in a partially depressed position, rather than return to the idle position. This could result in a loss of throttle control and a crash causing property damage and/or personal injury.
On certain vehicles, the accelerator pedal may become stuck in the wide open position due to an unsecured or incompatible driver's floor mat. A fully depressed accelerator pedal may result in very high vehicle speeds and make it difficult to slow or stop the vehicle. This could result in a crash causing property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will modify or replace the accelerator pedal.
On certain vehicles, the interior housing embossment retaining the interior door handle spring may fracture during normal usage, resulting in insufficient spring force to return the handle to the fully stowed position. The door latch will remain fully engaged, however, the handle may be loose, causing a rattle or noise, and the handle may not return to the fully closed position. Depending on spring load force and handle position, the inertial loads experienced by the interior door handle may cause the door latch to open during a side impact crash. If this were to occur and the door opened during a crash, an occupant not wearing a seatbelt could be ejected from the vehicle, increasing the risk of personal injuries.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and, if necessary, either add a spring embossment reinforcement or replace the interior door handle assembly.
On certain Sprinter 3500 vehicles, incorrect A-pillar interior trim may have been installed during vehicle assembly. As such, the vehicle may not comply with the requirements of Canada Motor Vehicle Safety Standard 201 - Occupant Protection. Installation of incorrect A-pillar trim could increase the risk of personal injuries during a crash.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and, if necessary, replace the A-pillar trim panels.
On certain trucks equipped with an air release fifth wheel hitch, the inversion valve on the frame may be orientated where contaminants can enter the valve through the exhaust port. As a result, the brake valve may corrode causing it to malfunction and in some cases result in unintended opening of the fifth wheel's locking mechanism. This could allow the trailer to separate from the towing vehicle while underway. Also, an air loss could increase vehicle stopping distance and result in a crash causing property damage or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and replace the inversion valve if required.
Certain vehicles may have control module settings that disable the locking function for the liftgate. Technical Standards Document No. 206 S4.3 requires that that each door, including liftgates, be equipped with a locking device.
On certain vehicles, the hood ornament's retracting mechanism may become corroded. This may lead to the hood ornament not retracting as intended when a vehicle is involved in a crash, and may increase the risk of injury to a pedestrian if struck.
On certain vehicles, heat created by the engine could cause possible deformation of the rear right hand and left hand wheelhouse and heat shield assemblies. If not remedied, the assembly could become excessively close to the exhaust system causing the bonding adhesive used in the assembly to overheat and produce smoke, or in remote circumstances under particularly high ambient operating temperature conditions cause the adhesive to catch fire, also rendering the vehicle inoperable and possibly resulting in a crash.
On certain motorcycles, if the hand protector is twisted out of position relative to the handlebar lever, to the extent that lever and protector come into contact, there is a possibility that the handlebar lever remain continuously in the partly operated position. This is turn can cause problems with the brake function and could increase the risk of a crash.
Safety Risk
No risk description available.
Remedy
Owners will receive a loose-leaf insert as an addendum to the Rider's Manual and two stickers complete with the installation instructions, or owners may have their authorised BMW motorcycle dealers do the installations for them.
On certain trucks, the bottom track that attaches to the arms used to open and close the window may de-bond, resulting in the glass coming loose at the bottom. If this occurs and is left unresolved, the upper window hinges may fail and allow the glass to fall out of the window frame. A window which falls from the vehicle may blow into the air, presenting a risk of hitting other vehicles, a stationary object, or bystanders, causing property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and, if necessary, replace the window assembly.
On certain motorcycles, the center stand pivot screw may come loose during normal operation. If the screw backs out, the center stand may detach, presenting a ground clearance hazard, or coming into contact with the rear wheel and final drive assemblies. This could cause property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will apply thread locking coumpound to the center stand pivot screw.
On certain motorcycles, the right rear portion of the lower fairing can interfere with the operator's footwear while applying the rear brake. This could
diminish the operator's control over brake force application or distract the operator, creating the risk of a crash resulting in personal injury or death.
Safety Risk
No risk description available.
Remedy
Dealers will install an updated footrest guard and fairing bracket.

On certain vehicles, the headliner pushpin at the B-pillar may not absorb sufficient energy to conform to the U.S specific regulatory requirement for unbelted occupant protection in an interior impact. In the event of a crash, the headliner pushpin in this location may not provide the interior impact protection intended, increasing the risk of injury. Although Canadian vehicles comply with all Canada Motor Vehicle Safety Standards, Ford Motor Company of Canada is voluntarily conducting this customer satisfaction program to reassure Transit Connect owners.
Certain vehicles may fail to conform to Canada Motor Vehicle Safety Standard (CMVSS) 201 - Occupant Protection. The instrument panel center lower box door may not remain closed during a vehicle crash, increasing the risk of personal injury to the occupants.
Safety Risk
No risk description available.
Remedy
Dealers will replace the instrument panel center lower box with an updated version.
On certain motorcycles, the fuel tank mounting fasteners can work loose, allowing the bolts to slip toward the inboard side and contact the fork leg if the handlebar is turned a significant amount. This could limit handlebar travel range, affecting handling and maneuverability of the motorcycle, which could increase the risk of a crash causing property damage, personal injury or death.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and tighten the fuel tank mounting bolts as per specifications.
On certain vehicles equipped with power sliding doors, the sliding door wiring harness may have been routed improperly and chafe on the lower sliding door hinge bracket. This could result in a fire within the sliding door assembly, which could lead to property damage or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will inspect the sliding door wire track harness and power drive unit wiring harness assemblies for damage. The harnesses will be repaired or replaced as necessary, and rerouted under the sliding door weatherstrip.
On certain vehicles, the accelerator pedal pivot bearing bushings may dislodge from the pedal bushing bearing pockets and cause the accelerator pedal to bind or stick. Even though these vehicles are equipped with "Smart Brake technology", where the engine controller is designed to reduce engine power when a disagreement between the throttle and the brake exists, the risk of a crash causing property damage, personal injury or death still exist.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and, if necessary, replace the accelerator pedal assembly.
