On certain vehicles, the Electronic Control Module (ECM) software may not detect a brake pedal switch failure. If the brake pedal switch malfunctions, application of the brake pedal would fail to deactivate the cruise control. The driver would, however, be able to disengage the cruise control by other means such as; pressing the cruise control main switch located on the instrument panel, shifting the transmission into neutral or another gear, lowering the speed of the vehicle by more than 48 km/h (30 mph) through braking and/or shifting gears, or by turning the engine OFF. Failure or delay in disengaging the cruise control can increase the risk of a crash causing property damage and/or personal injury.
On certain vehicles, the passenger seat Occupant Classification System may malfunction and could misinterpret a small stature person as a child seat, thereby deactivating the passenger frontal airbag and illuminating the "PASSENGER AIRBAG OFF" warning light. This could result in the airbag not deploying when warranted in a crash, increasing the risk of personal injury. Note: Within 50 meters of driving distance, the occupant classification system would automatically re-evaluate the occupant classification and correct the issue.
Safety Risk
No risk description available.
Remedy
Dealers will verify the proper operation of the Occupant Classification System, and affect repairs if necessary.
On certain vehicles, due to a defect in the restraint control module programming, deployment of the side curtain airbags may be delayed in certain rollover circumstances, which could increase the risk of injury to vehicle occupants.
Safety Risk
No risk description available.
Remedy
Dealers will reprogram the restraint control module.

On certain vehicles, the reverse gear speed limiter may not meet design specifications and could allow the vehicle to travel at more than 25 km/h in reverse. Safety Risk
No risk description available.
Remedy
Dealers will update the engine control unit software to limit the speed while travelling in reverse.

Safety Risk
IN THE EVENT OF A CRASH, THE SEAT OCCUPANT MAY NOT BE PROPERLY RESTRAINED, INCREASING THE RISK OF INJURY.
Remedy
DEALERS WILL HAVE THE DRIVER AND FRONT PASSENGER SAFETY BELT BUCKLE COVER REMOVED AND REPLACED WITH A NEWLY DESIGNED SERVICE BUCKLE COVER. OWNER NOTIFICATION BEGAN FEBRUARY 5, 2004. OWNERS SHOULD CONTACT MAZDA AT 1-800-222-5500.
