On certain 2500 and 3500 series heavy duty pickup trucks equipped with a Cummins diesel engine and a manual transmission, an elevated idle speed may occur after extended use of the cruise control. The engine will run at this elevated idle speed until either key off, or until the accelerator pedal is nearly fully depressed. The customer perception is that the cruise control will not disengage or that the accelerator linkage is sticking.
Safety Risk
No risk description available.
Remedy
Dealer will reprogram the software in the Cummins engine controller.

THE PARKING BRAKE CONTROL SELF-ADJUST PAWL MAY SKIP OVER ONE OR MORE TEETH IN THE SELF-ADJUST RATCHET DURING PARKING BRAKE APPLICATION, WHICH COULD PREVENT THE SYSTEM FROM ACHIEVING FULL TENSION. THIS COULD RESULT IN PARKING BRAKE INEFFECTIVENESS WHICH WOULD ALLOW THE VEHICLE TO ROLL FREELY IF IT IS NOT LEFT IN GEAR.
Safety Risk
No risk description available.
Remedy
A WEDGE WILL BE INSTALLED WHICH WILL SERVE TO LOCK THE PARKING BRAKE SELF-ADJUST MECHANISM PAWL INTO ENGAGEMENT WITH THE RATCHET.

THE ELECTRICAL CABLES FROM THE BATTERY TO THE STARTER SOLENOID COULD BECOME WEDGED BETWEEN THE BOTTOM OF THE SEAT AND THE WIRE GUIDE. THIS COULD LEAD TO INSULATION DAMAGE AND A POSSIBLE SHORT CIRCUIT WHICH COULD IN TURN CAUSE ENGINE STOPPAGE AND LIGHT FAILURE, RESULTING IN A POSSIBLE ACCIDENT.
Safety Risk
No risk description available.
Remedy
CABLES WILL BE ROUTED THROUGH THE COIL BRACKET, A GROMMET WILL BE INSTALLED TO PREVENT CHAFING OF THE INSULATION AND CABLES WILL BE SECURED WITH A PLASTIC TIE WRAP. CABLES FOUND TO BE DAMAGED DURING INITIAL INSPECTION WILL BE REPLACED.
Certain vehicles fail to comply with the requirements of Canada Motor Vehicle Safety Standard 108 - Lighting System and Retroreflective Devices. An incorrectly manufactured turn signal lever could cause the auto-cancel function to become inoperative. The vehicle operator will, however, continue to be able to manually cancel the turn signals.
Safety Risk
No risk description available.
Remedy
Dealers will inspect and, if necessary, replace the turn signal lever.
POSSIBLE INSUFFICIENT CLEARANCE BEWTWEEN THE FUEL TANK VENT TUBE AND VEHICLE FLOORPAN WHERE THE TUBE PROJECTS THROUGH AN ACCESS HOLE INTO THE TOOL COMPARTMENT UNDER THE TRUNK FLOOR. THE VENT TUBE MAY CONTACT THE OPENING AND BECOME DAMAGED RESULTING IN FUEL LEAKAGE.
On certain 318I, 318IC and 318IS vehicles, at temperatures below -10 degrees centigrade with the engine partially or completely warmed up, condensation from the crankcase ventilation system may freeze at the throttle housing and prevent the throttle plate from fully closing when the accelerator pedal is released. This could result in a slow rate of deceloeration from speed or increased idle speed both of which may adversely affect vehicle control.
Safety Risk
No risk description available.
Remedy
Throttle house will be replaced with a version containing a modified heating system and the crankcase vent will be rerouted.
On certain vehicles, if the driver's all-weather floor mat is not secured by the retaining clips, or is placed on top of an existing carpeted floor mat, the mat could move forward and interfere with the accelerator pedal. If this occurs, the accelerator pedal could become stuck in the wide open position. A fully depressed accelerator pedal may result in very high vehicle speeds and make it difficult to slow or stop the vehicle. This could result in a crash causing property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will modify or replace the accelerator pedal, as well as the genuine Lexus all-weather floor mat.

On certain vehicles, the driver frontal airbag inflator could produce excessive internal pressure during airbag deployment. Increased pressure may cause the inflator to rupture, which could allow fragments to be propelled toward vehicle occupants, increasing the risk of injury. This could also damage the airbag module, which could prevent proper deployment. Failure of the airbag to fully deploy during a crash (where deployment is warranted) could increase the risk of personal injury to the seat occupant.
Safety Risk
No risk description available.
Remedy
Dealers will replace the front driver air bag module.
On certain vehicles equipped with a 2.3L engine, the fuel line in the area of the throttle body shield may not have the protective convolute properly installed. If the protective convolute on the fuel line is not assembled in the proper position, the fuel line may contact the edge of the shield. Over time this contact may chafe the fuel line, resulting in a fuel leak. Fuel leakage, in the presence of an ignition source, could result in a fire causing property damage and/or personal injury.
Safety Risk
No risk description available.
Remedy
Dealers will trim the throttle body shield, inspect the fuel line for damage, and replace the line if necessary.
